KVKK Application Form and Procedure

Pursuant to Articles 11 and 13 of Turkish Law No. 6698 ("KVKK") and the Communiqué on the Procedures and Principles of Application to the Data Controller, you may submit requests concerning your personal data to the data controller Azure Private Dining ([COMPANY LEGAL NAME]) as follows.

1. Your Rights under Article 11 KVKK

  1. To learn whether your personal data are processed;
  2. to request information if they have been processed;
  3. to learn the purpose of processing and whether the data are used in line with that purpose;
  4. to know the third parties to whom the data are transferred, in Türkiye or abroad;
  5. to request rectification of incomplete or inaccurate data;
  6. to request erasure or destruction of the data under the conditions of Article 7 KVKK;
  7. to request that rectification/erasure/destruction be notified to third parties to whom the data were transferred;
  8. to object to a result arising to your detriment through analysis exclusively by automated systems;
  9. to claim compensation for damage arising from unlawful processing.

2. Application Channels

Applications must be submitted in Turkish through one of the following channels:

3. Mandatory Content of the Application (Communiqué Art. 5/2)

  • Name, surname and (for written applications) signature;
  • Turkish ID number for Turkish citizens; nationality and passport number (or ID number, if any) for foreigners;
  • Residential or workplace address for notification;
  • E-mail address, telephone or fax number for notification, if available;
  • Subject of the request.

Information and documents relating to the request must be attached. Applications on behalf of another person require a power of attorney containing special authorisation or proof of legal representation.

4. Response Time and Fees

  • Your application will be concluded as soon as possible and within 30 (thirty) days at the latest, depending on its nature, and the response will be notified to you in writing or electronically.
  • Applications are in principle free of charge; where the process entails an additional cost, the fee in the tariff determined by the Personal Data Protection Board may be charged. If the application results from an error on our part, any fee collected will be refunded.
  • Your request will be granted, or refused with reasons. If your application is refused, if you find the response insufficient, or if no response is given in time, you may lodge a complaint with the Personal Data Protection Board within 30 days of learning of the response and in any event within 60 days of the application date (Article 14 KVKK).

Additional information or documents may be requested to verify that the application belongs to you. Such information is processed solely for identity verification and for concluding the application, and is retained for the periods required by the applicable legislation.
